Description
Participants under 18 must obtain a parent or guardian?s signature, and those under 16 must be accompanied by a parent or guardian while working out. Prior to participation at the Fitness Center, all members must fill out & sign a health information medical screening question-naire and obtain a physician?s signature if deemed necessary.
